Referral criteria
Conditions Treated:
Familial Hypercholesterolaemia (FH)
Other genetic hyperlipidaemias eg Type I and Type III
Mixed hyperlipidaemia
Patients requiring combination therapy for lipid lowering
Patients experiencing problems with lipid lowering medication
Procedures Performed:
1. Consultation
2. Further investigations as appropriate
3. Diagnosis
4. Management plan and referral to dietitian as appropriate
Exclusions:
Age less than 16
Required Investigations:
Fasting (usually) blood sample 1 week prior to clinic
Lipids, U+E, glucose, LFT, CK, TSH, fT4, FBC (recent HbA1c if diabetic)
Administrative Requirements:
1. Patient’s full demographics
2. Previous history and family history
3. Present drug therapy and known allergies (and, where necessary, information about the use of previous lipid lowing medication and details of any problems encountered)
4. Transport requirements
